We now offer a low-cost, in-office dental plan with a nominal membership fee. Our plan includes preventive care and discounts on restorative treatments, such as fillings and crowns, allowing you to maintain a healthy smile affordably.

Please note, this plan does not come with a membership card as it is specific to our office. We will keep your effective date on file for easy reference.

Program Guidelines

• Membership fees are due on the day of joining.
• Membership expires one year from the join date.
• Refunds of premiums will not be issued.

Dental Exclusions & Limitations

This program is a discount plan, not dental insurance. It cannot be used:

• In conjunction with any other dental plan or insurance
• For costs covered under automobile or medical insurance
• For injuries covered under worker’s compensation
• For treatments referred to a specialist
